Reducing disparities in pneumococcal vaccinations related to geography and to provider performance within the Veterans Health Administration (VHA) of Ohio

The overall goal for this project is to support implementation and use of the electronic clinical reminder as a means to increase immunizations againsts pneumococcal disease among eligible veterans.
