GOAL: Initiation of anticoagulation for patients with venous thromboembolism (VTE) newly diagnosed in the hospital is an important patient safety threat.  A multicomponent intervention including pharmacist inpatient consultation to choose between new oral anticoagulant versus warfarin, improvement of discharge medication instructions with illustrations, coordination of care across all settings, and pharmacist home visit holds the promise of better quality transition for the patient with VTE.
